Specific instructions are on the first page, please take a moment and read through them. This first survey is a broad look at Airedale health; we are hoping to conduct 2 more surveys over the next year exploring issues from the first survey. IF your dog(s) have (had) health problems that are not included on this first survey there is a place at the conclusion of the survey for you to write in those problems so we can include them in the follow up survey.
